Punjab NRHM invites Online applications for the following posts on contract basis :

1. Staff Nurse : 134  posts, Age : 18-37 years, Pay : Rs. 8500 + Rs. 1500 (Staying Allowance)  at CHCs and Rs. 7500 + Rs. 1000 (Staying Allowance) at PHCs
2. ANM  : 437 posts, Age : 18-45 years, Pay : Rs. 6900

Application Fee : Demand Draft of Rs.300/- (Rs.150/- for reserved categories)  in favor of "State Health Society, Punjab" payable at Chandigarh 

How to Apply : Apply Online at CDAC website upto 22/12/2010.  Get the printouts (2 copies) of the filled-in online form Registration slip bearing Registration Number and other details of the candidate as filled in by him/ her online.  Send/Post one copy of the Online Registration Slip (Printed) with a self attested photograph along with the original crossed Demand Draft of requisite fee (non-refundable) at C-DAC Mohali, A-34, Phase VIII Industrial Area, Mohali  on or before 26/12/2010.
